The purpose of this program is to provide diabetes treatment and/or prevention activities and/or services (also referred to as “activities/services”) for American Indian / Alaska Native (AI/AN) communities. Awardees will implement one Special Diabetes Program for Indians (SDPI) Diabetes Best Practice (also referred to as “Best Practice”) and report data on the Best Practice’s Required Key Measure (RKM). Awardees may also implement other activities/services based on diabetes-related community needs and develop an evaluation plan. Activities/services will be aimed at reducing the risk of diabetes in at-risk individuals, providing high quality care to those with diagnosed diabetes, and/or reducing the complications of diabetes.
