This notice announces the opportunity to apply for funding under the Comprehensive Systems Integration for Adolescent and Young Adult Health (CSI-AYAH) program. The purpose of this program is to increase the capacity of states, territories and tribal organizations to integrate systems of care, including health,1 school,2 and community3 systems, to promote adolescent and young adult (youth) health and well-being. This program will: 1) Establish a cross-sector alliance comprised of nationally recognized partners in health, school, and community systems that will collectively provide national leadership; 2) Support the alliance to develop and disseminate a national blueprint to improve adolescent health and well-being through systems integration in states, territories, and tribal organizations; 3) Provide targeted technical assistance (TA) to learning collaboratives comprised of leadership teams in states, territories, and/or tribal organizations working to implement cross-sector systems integration models; 4) Deliver TA to support Title V agencies that have selected performance measures related to improving adolescent health and/or behavioral health; and 5) Increase the number of state, territory, and tribal organizations accessing and implementing systems integration models to improve youth health and well-being.
