The purpose of this initiative is to advance hematopoietic cell transplantation (HCT) for all patients, but particularly for patients with rare and difficult to treat non-malignant blood diseases and hematological malignancies. This will be accomplished by supporting a clinical trials network to evaluate novel cell therapy (CT) and HCT approaches. A Data Coordinating Center (DCC) and core clinical sites will be supported by funds requested as part of this initiative, as well as a limited number of trials. Additional studies and trials will be funded through collaborations with RO I-funded grantees, industry, and foundations. Multiple factors extend the duration of most HCT and CT trials beyond 5 years, including rare patient populations (and hence accrual periods of up to 5 years), complex cell manufacturing processes, and composite endpoints such as disease-free survival and chronic graft versus-host disease free survival that occur 2 or more years post-transplant. A seven year program to support these activities is needed.
