The purpose of Healthy Start Enhanced (HSE) is to improve health outcomes before, during, and after pregnancy and reduce racial/ethnic differences in rates of infant death and adverse perinatal outcomes. The program uses two approaches to accelerate disparity reduction and improve maternal and infant health outcomes: 1) providing direct and enabling services to HSE participants; and 2) establishing Community Action Networks (CANs) comprised of diverse, multi-sector partners. The CANs develop and implement action plans to advance equity by addressing the unique SDOH contributing to disparities in poor birth outcomes within the project area. HSE sites will participate in CAN-focused learning communities, facilitated by the Healthy Start Technical Assistance and Support Center, on SDOH topics such as housing, food insecurity, and transportation.MCHB
