The CDC’s National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) announces the availability of funds to state, local, territorial, and tribal jurisdictions, that are ready to take ACTion (Addressing Conditions To improve population health) in communities to implement policy, systems, and environmental change (PSE) interventions that address Social Determinants of Health (SDOH) to reduce chronic disease related disparities, risk factors, and inequities. This NOFO will incorporate the lessons learned from the fiscal year (FY) 21 (DP21-2111) and FY 22 (DP22-2210) SDOH Accelerator Grants to fund pilot ACTion projects focused on implementing established plans that include PSE interventions across four domains; the built environment, community-clinical linkages, food and nutrition security, and social connectedness. Pilot ACTion project activities will include engaging multisectoral partnerships to implement and evaluate PSE interventions that address one of the four SDOH domains. Applicants proposed ACTion pilot project applications must:Demonstrate they are ready to take ACTion by:Including an established plan ready for implementation that describes specific PSE interventions that will address at least one of the four NCCDPHP SDOH domains of focus.Describing an established leadership team that will engage multisectoral partners to guide, implement, and evaluate their proposed ACTion project.Demonstrate capacity to develop and carry out an evaluation plan for their ACTion project. Demonstrate how their proposed pilot ACTion project will improve the health of Medicaid beneficiaries in their community.
